Insurance Information
The North Suburban Library System offers a group insurance program to assist libraries in obtaining affordable insurance. The program is open to all Illinois public libraries, public library districts, and library systems.
The program offers group coverage for:
- health insurance
- vision reimbursement benefit of $200 every 24 months – no network
- dental insurance
- life insurance & accidental death insurance
- long-term disability insurance
- short term disability insurance
- Section 125 Plan

Health Insurance
The health insurance is a partially self-funded P.P.O. plan and is administered by NSLS.
Provider Network
- We offer the Aetna Signature Network, which is “national” and offers a wide variety of choices with extensive coverage throughout the United States.
- Our Prescription Drug Program Administration is covered by Caremark.
Claim Processing
The claims are processed by Allied Benefit Systems, Inc. located in Chicago, Illinois. Allied offers online access to claims and claims history.

Vision Benefit
The plan will reimburse up to $200 for vision services during a 24 month period. Go to the vision care provider of your choice, then submit your claim for reimbursement.
Wellness Benefits
Staying healthy and living a healthier lifestyle are a high priority for our program. We believe that in order to provide the best possible value, we need to offer a wellness program that is proactive. Wellness benefits include:
- Annual Routine Physical - $500 maximum per calendar year with plan level co-pay;
- Well Child Care to 18th birthday - $500 maximum per calendar year with plan level co-pay. Includes examinations, immunizations, x-rays and lab tests;
- Mammogram, Pap smear, prostate blood tests and fecal occult screenings - One per calendar year, covered at 100% deductible waived; and
- Routine Colonoscopy - $400 co-pay then covered at 100%; In-Network Only.
The plan includes convenient annual Health Screenings at various library locations. The screenings include:
- Health Profile blood test – 35 tests including CBC, cholesterol analysis and many more of the major areas of concern;
- Blood pressure evaluation;
- Body Mass Index evaluation;
- Flu Shot; and
- Health Power Profile evaluation and report with online access.
Additional tests that may be included are PSA, Thyroid Screen, Vitamin D Deficiency Screen, and Bone Density Screen. Detailed test information will be available in August.

Dental Insurance
North Suburban Library System’s Dental Plan does not participate in a network of dentists, so members may see the dentist of their choice. View detailed dental plan information beginning on page 36 of the plan document. Claims are processed by Allied Benefit Systems, Inc. located in Chicago, Illinois. Allied offers online access to claims and claims history.
Cobra Administration
The North Suburban Library System Group Insurance Plan pays for COBRA administration through Allied Benefit Systems. The COBRA laws are complicated and Allied Benefit Systems has attorneys who know how to handle any situation.
Section 125 Plan
Our Section 125 Plan gives employees the opportunity to pay for their Health and Dental Insurance premiums, Dependent Care Expenses and qualified medical/dental expenses using pre-tax dollars. This program also saves employers the Social Security matching due to lowered gross earnings. The fees for this service are:
- $125 - Annual Administration Fee per library.
- One-time setup fee per library: $250 if in the NSLS Health Plan or $450 if not in the NSLS Health Plan.
- $8.00/employee/month for employees enrolled in the Flexible Spending Plan ONLY.
There is no additional monthly charge for the P.O.P. Plan, beyond the annual fee and one-time setup fee. View additional information about the Section 125 Plan.
Utilizes an auto-interface between the medical and dental plans for fast and easy reimbursement of flex spending, with a direct deposit into your bank account and email notification.
Life, Long Term Disability & Short Term Disability
Jefferson Pilot/Lincoln Financial Group issues the following policies:
- Life & Accidental Death Insurance: 1 x’s salary, to a maximum of $50,000/year
- Long Term Disability Insurance: wages up to $60,000 per year.
- Short Term Disability Insurance: wages up to $59,428 per year.

Special Benefits and Plan Administration
Our plan has been enhanced to provide greater ease in the administration of the plan, as well as processing by beneficiaries.
- No library is too small (or big) for the program. This is a rare opportunity for small libraries to option affordable insurance.
- One consolidated billing each month for all of the insurance programs that you participate in.
- Online access to health, dental and flex claim accounts so that members may view their claims anytime.
- Flexible spending account with an auto-interface between the medical and dental plans for fast and easy reimbursement of flex spending, with a direct deposit into your bank account and email notification.
- COBRA Administration included in your monthly premiums. COBRA laws are complicated. Our plan lets the professionals at Allied Benefit Systems, Inc. take care of your COBRA administration needs.
- Our plan will schedule employee meetings at your library to educate your staff on the plan including online demos with valuable information and resources.
- Wellness programs that your library may participate in to promote healthy living. Over the past six years, we have seen our members participate in the health screenings and the reward is healthier staffs at the libraries. By promoting wellness, productivity increases and staff are more engaged at work.
